Thank you for your interest in seeking advice from Advent Legal in relation to the drafting of your Power of Attorney

To allow us to better understand your requests and requirements, please fill in the intake form as completely as you can, and upload the following:

  • 2 copies of ID (both sides) for yourself and your designated attorney(s). One piece of ID must be Canadian/Federal or Provincial government-issued photo identification such as a driver’s licence or passport. Unfortunately, a health card is not acceptable identification;
  • A copy of any previous power of attorney documents

You can specify whether or not you would like to use a standard attorney where your attorney has full powers to act on your behalf or if you would like to limit some powers (for example, you can limit if your attorney can sell property on your behalf).

As part of the Power of Attorney for Personal Care, we prepare a Health Care Directive which helps family members and medical professionals understand your choice for end of life care and whether you wish to have heroic measures used to keep you alive if you are only surviving as result of machines. In this scenario, you have no hope of recovery and are only being kept alive by machines such as a respirator. If you would like to speak with us before making a decision, please skip this question.
